New Hire Requirements
- California Driver's License
- Social Security Card
- Work Authorization, if alien
- CPR Card (class A, B, C)
- Health Examination (including a doctor's statement that you are physically able to perform duties required, free of communicable diseases, and a chest x-ray or TB skin test)
- CNA Certificate
- Work References (2 or more)
- Criminal Background Check or Live Scan (must include an ATI # and a copy of your fingerprints)
- DMV Report
- Auto Insurance (copy of policy)
- Malpractice Insurance (applications available in office if needed)
